Appointments
We see patients in our office on Mondays and Wednesdays, by appointment only. We do require referral from your family physician or another specialist. Call 817-465-7764 to schedule an appointment.

What to Bring to your Appointment
To schedule an online appointment you will need to bring any x-rays, diagnostic studies, or medical reports regarding your condition, complete our Patient Medical History Form, and make sure to have your ID and Insurance Card with you at your appointment.

Patient Medical History Forms
To schedule an appointment you must first complete our new patient forms online and submit them electronically to us. This will eliminate the need to complete the forms at the office and will help ensure your appointment is seamless from check-in to check-out.
What Information is Most Important to the Doctor during your Visit?
While many patients want to discuss in great detail the exact event which may have caused their injury, the most important information your health care team needs involves your symptoms (such as where you have pain, weakness, numbness, etc.) and what limitations these symptoms place on your everyday activities. Please take the time to write down this information so you can share it with us. We encourage you to write down all of your other concerns as well so you will not forget to ask any important questions.
Insurance
We accept most commercial insurances, as well as Medicare, Medicaid, Texas and federal worker’s compensation. You will be required to have your insurance card and co-pay available at the time of your visit. We will assist you with the billing of your primary insurance. Letters of protection are not accepted.
If your insurance stipulates that you must have a referral to see us, be sure that your doctor has sent us the necessary information. We are not able to see you without this referral. Also, please review your medical insurance coverage regarding your benefits, covered services, any exclusion, limitations, deductibles or copayments, and the rules you need to follow as stated in your Evidence of Coverage.
You will be required to pay your co-payments at the time of your appointment.
If surgery is indicated upon initial evaluation, all FMLA and disability forms must be presented to our office in a timely fashion. Please allow 7 to 10 working days for form completion. There will be a $40.00 charge per form for our office to process FMLA and disability forms.
Authorizations
It is your responsibility to ensure whether a service is covered on your insurance plan. You may need to contact your insurance company before making the appointment to determine this.

Prescriptions and refills
Prescriptions are issued as needed at the time of your visit. Refills are accepted during office hours only. Please ask your pharmacist to contact our office for appropriate refills.
Emergencies
In the event of a medical emergency please call 911 or proceed to the nearest emergency room. The ER staff will evaluate you, and notify the on-call surgeon.
